fa مروری بر عملکرد مقیاس WSES 2020 و مدل‌های یادگیری ماشینی برای پیش‌بینی نتایج بالینی در آسیب‌های تروماتیک کبد؛ یک مطالعه ثانویه با رویکرد آموزشی A review of the performance of the WSES 2020 scale and machine learning models for predicting clinical outcomes in traumatic liver injuries; a secondary study with an educational approach تخصصي Special مروری Review مقدمه: طبقه‌بندی آسیب کبدی 2020 انجمن جهانی جراحی اورژانس (World Society of Emergency Surgery – WSES) یکی از آخرین گایدلاین هایی است که شدت آسیب آناتومیکی را با وضعیت همودینامیک ترکیب می‌کند که در این مطالعه عملکرد آن نسبت به روش‌های مدرن یادگیری ماشین مقایسه می شود. برای درک متغیر های پر اهمیت، از روش SHapley Additive exPlanations استفاده شد. روش کار: این مطالعه ثانویه داده های مطالعه ای گذشته ‌نگر می باشد. هشت الگوریتم یادگیری ماشین نظارت‌شده شامل XGBoost، LightGBM، CatBoost، Random Forest و Logistic Regression آموزش داده شدند تا هر پیامد را پیش‌بینی کنند و عملکرد آن‌ها با قانون WSES با استفاده از AUC-ROC مقایسه شد. نتایج: مدل‌های ML برای پیش‌بینی نیاز به جراحی (CatBoost AUC 0.833 در مقابل 0.573، Δ +0.260، p < 0.001) عوارض کبدی (Logistic Regression AUC 0.722 در مقابل 0.549، Δ +0.173، p = 0.012) عوارض جدی (CatBoost AUC 0.809 در مقابل 0.622، Δ +0.187، p < 0.001) و آنژوامبولیزاسیون/لاپاراتومی مجدد (LightGBM AUC 0.833 در مقابل 0.598، Δ +0.235، p < 0.001) عملکرد بهتری داشتند. نتیجه‌گیری:  طبقه‌بندی آسیب کبدی WSES 2020به خوبی مرگ و میر در بیمارستان را پیش بینی می کند اما نسبت به مدل‌های ML در پیش‌بینی نیاز به جراحی و عوارض، عملکرد ضعیف‌تری دارد.   Introduction: The 2020 World Society of Emergency Surgery (WSES) Liver Injury Classification is one of the latest guidelines that combines the severity of anatomical injury with hemodynamic status, and in this study its performance is compared with modern machine learning methods. To understand the variables of interest, the SHapley Additive exPlanations method was used. Methods: This secondary study analyzed retrospective study. Eight supervised machine learning algorithms including XGBoost, LightGBM, CatBoost, Random Forest, and Logistic Regression were trained to predict each outcome, and their performance was compared with the WSES rule using AUC-ROC. Interpretation of the models was performed with SHAP values. Results:  ML models performed better for predicting the need for surgery (CatBoost AUC 0.833 vs 0.573, Δ +0.260, p < 0.001), hepatic complications (Logistic Regression AUC 0.722 vs 0.549, Δ +0.173, p = 0.012), serious complications (CatBoost AUC 0.809 vs 0.622, Δ +0.187, p < 0.001), and angioembolization/re-laparotomy (LightGBM AUC 0.833 vs 0.598, Δ +0.235, p < 0.001). SHAP analysis identified hemodynamic instability, injury severity scores, and associated abdominal injuries as the most important predictors.
